-[ncam] Better solution for setting CBC mode for CSA
(Instead of hard coding the algo mode in the dvbapi_write_cw(), we now initialize the er->cw_ex variable with the proper value)
-Simpler code for the EXTENDED_CW struct
-[ncam] Renamed caid_is_biss_static() to caid_is_biss_fixed()
